As far as the personal data provided by the user, including when visiting this website, POLIAMBULATORIO SAN GAETANO S.R.L. has appointed Mr. Luca Zancan as the person responsible for their treatment.
The data we collect will not be disseminated. However, in order to provide the service requested, some data will be shared with external parties asked to carry out specific tasks on behalf of our company (e.g. healthcare personnel other than doctors, independent doctors, administrative personnel, web agencies, marketing consultants). The data controller undertakes to ensure the security of personal data by taking all the necessary IT and physical measures to protect the personal data you provide. No security system can however guarantee full and certain protection; therefore, without prejudice to any liability due to negligence of the data controller, POLIAMBULATORIO SAN GAETANO S.R.L. shall not be held responsible for any actions carried out by third parties who might access the systems without the necessary authorisations.
List of the Main Personal Data Processing Activities on the Website.
1) This website does not include a user registration procedure. No personal data are obtained if you simply browse the website. The use of cookies does however allow us to collect information that the user does not provide directly. This data is stored with computerised systems for a specific period of time, that depends of the type of cookies installed (see Cookie Policy);
2) By filling the “BOOKING AND INFORMATION REQUEST” contact form (available in the “Poliambulatorio” (Medical Care), “Medicina Estetica” (Aesthetic Medicine), “Studio Dentistico” (Dental Surgery) sections and in any other sections for which the above window appears), the user provides their personal data, some of which are sensitive due to their health connotations (name, email, telephone, service required, explanation of the problem, contact method and message). The provision of the data is not regulated by any legal or contractual requirements and is therefore completely voluntary. However, failure to provide the requested data will prevent the company from supplying the information and the services requested by the user. The period of conservation of the data expires when the information requested is provided, unless the user decides to book the service. In this case, the data will be kept in order to fulfil the healthcare request (e.g. medical examination) and the period of duration of the treatment will be ten years, in compliance with the law and to enable the fulfilment of the accounting and fiscal requirements. In case of online booking and failure to continue with the healthcare service, the data supplied with the sole online booking shall be deleted after the cancellation notification by the user.
It is pointed out that the booking does not entail the stipulation of a healthcare treatment contract with the company, and that the same shall only be formalised upon arrival of the Customer at the Poliambulatorio for the provision of the healthcare service.
Requests of information and bookings can only be accepted from users that are 18 years old or over.
2.1) The data supplied with the above contact form may also be used to forward commercial advertising material. In this case, the period during which the data will be kept shall depend on the user, who shall be entitled to withdraw at any time their consent by clicking the “delete me” button on the communication itself, or by directly contacting the company to request the cancellation of the service (at the addresses indicated in the general information sections).
Subscription to the Newsletter is only allowed to users that are 16 years old and over or, if younger, subject to authorisation of their legal guardians.
3) By filling the appropriate contact form, in the “ONLINE REPORT” section the user enters their name and password received at the moment of booking (not online) or when the service was provided. The provision of the data is not a legal or contractual obligation. The provision of the data is therefore completely voluntary. However failure to provide the requested data will prevent the company from providing the service, consisting in the possibility of downloading the “report” (intended as the result of the medical or instrumental examination carried out) and the “report of findings” issued by the doctor (intended as the written report issued by the doctor on the clinical condition of the patient following a clinical or instrumental examination).
It is pointed out that the consent to the treatment of the data for the purpose of the download of the report or report of findings was provided by the customer at the moment of the booking (not online) or execution of the examination.
Reports shall be kept online for a period not exceeding 45 days.
3) Without prejudice to the regulations on cookies (which should be read for more information), this website does not provide an automated decision-making process for business profiling purposes (targeted marketing technology that includes the collection and processing of user data in order to understand the choices and behaviour of users, in order to subdivide their interests into “profiles”, or homogeneous groups by more specific behaviour or characteristics).
